Asian Community Care Management (ACCM) is Hiring!

ACCM is actively seeking a compassionate and experienced Social Worker/Care Manager (LMSW/MSW/RN) to join our dedicated team. As an approved Utilization Review Agent under the New York State Department of Health, ACCM is committed to providing excellent care management services to our members.

Responsibilities:

  • Conducts monthly outreach to members to check on their health status and needs
  • Develops and updates individualized care plans based on the comprehensive UAS assessment and communication with members and/or caregivers
  • Facilitates care planning as part of the interdisciplinary team
  • Performs psychosocial assessment, counseling, and crisis intervention
  • Promotes community resources and makes necessary referrals
  • Coordinates care transitions, including discharge and transition planning from the hospital or nursing homes
  • Conducts ongoing communication and collaboration with members, families, as well as with members’ PCP and other important health care providers
  • Arranges and coordinates delivery of needed services to all members
  • Provides monthly health and/or mental health education to members and families
  • Monitors member’s functional status and progress towards established care plan goals and adjusts when needed
  • Updates case records timely and maintains documentation accuracy
  • Advocates for members and families in need of services to better their health
  • Makes appropriate abuse referrals to Adult Protective Services when needed – If any abuse, neglect, and/or exploitation is suspected, report to authorities as obligated by law
  • Arranges Hospice and Palliative Care
  • Assists members with Medicaid-related issues, such as Medicaid renewal and PA benefits
  • Identifies and analyzes members’ strengths, deficits, risks, and available resources to enhance and promote members’ quality of life
  • Participates in regular staff meetings and training events
  • Performs other duties as assigned

Fundamental Components:

  • Application and/or interpretation of applicable criteria and clinical guidelines, standardized care management plans, policies, procedures, and regulatory standards while assessing benefits and/or members’ needs to ensure the appropriate administration of benefits.
  • Using a holistic approach, consults with care management colleagues, supervisors, clinical directors, and/or other programs to overcome barriers to meet goals and objectives.
  • Identifies and escalates quality of care issues through established channels.
  • Ability to speak to medical and behavioral health professionals to influence appropriate member care.
  • Provides coaching, information, and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
  • Helps members actively and knowledgeably participate with their provider in healthcare decision-making.
  • Analyzes all utilization, self-report, and clinical data available to consolidate information and begin to identify comprehensive member needs.

Job Type: Full-time

Pay: $70,000.00 – $80,000.00 per year
